Individual Education Plan |
An Individual Education Plan (IEP) is a document which describes all
special education services, related services, and supplementary aids
that a child needs to gain benefit from his/her educational program.
The IEP contains information about:
· The child’s levels of educational performance
· Measurable annual goals that will enable the child to progress in the general curriculum if appropriate
· Projected dates for accomplishment of short term objectives leading to the goals
· Special education and related services to be provided; including the frequency, location, and duration of those services
· Projected date for the beginning of services
· A description of the general characteristics of the child’s program
· Information regarding assessment; including State or district-wide testing
· A description of extended year services if required
· A statement about transition services for students 16 years or older
